All services are available through the NYS DOH TBI/NHTD Waiver Program, private pay, long term care insurance and worker’s compensation. We accept referrals from hospitals, inpatient facilities, family members, social workers and more. The individual must have a medical order from their primary care physician in order to receive services.

Service Coordination/Case Management

The overall success to individual choice and achievement is person-centered service coordination. The Service Coordinator:

  • Key pivotal individual to coordinate community based services
  • Coordinates and monitors the provision of all services
  • Assists in organization of daily living supports and services to meet the individual's needs and goals
  • Supports the participant's independence and integration into the community as well as maintaining the health and welfare of the individual

Independent Living Skills Training

It is the responsibility of the ILST provider to identify the participant's strengths/challenges in performing Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) related to his/her established goals.

Development of strategies to assist in the following areas:

Home Aide Services

Self-Direct, Inc. is a New York State Licensed Home Care Services Agency (LHCSA) and NYS certified Woman in Business Enterprise (WBE) that provides certified aides for Central New York including Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Cayuga and Cortland counties.

Home Health Aides (HHA)
Provides a higher level of care than PCAs including:

  • Medication Management/Administration
  • Wound care
  • Personal care tasks

Homemakers or Companions

Self-Direct, Inc. is a New York State Licensed Home Care Services Agency (LHCSA) that provides Companions and Homemakers for the elderly and disabled in Central New York including: Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Cayuga and Cortland counties.

Our Companions (also known as Homemakers) assist with meal prep, light household tasks, accompaniment in the home and/or community, and more. Companions are not certified by the NYS Department of Health although Self-Direct, Inc. does perform thorough background checks and training for all staff.

Our RNs are licensed and certified in New York State, and provide the highest level of Home Care Services which include:

Positive Behavioral Intervention Services

Individual interventions designed to decrease the intensity or frequency of targeted behaviors, and to teach more socially appropriate behaviors which, if not modified, will interfere with the individual’s ability to remain integrated in the community. PBIS is provided in the home and/or community to the individual, his/her family or others having close contact with the individual.

Community Integration Counseling

Community Integration Counseling is an individually designed service intended to assist participants who are experiencing significant problems in managing the emotional difficulties inherent in adjusting to a significant limitation and living in the community. It is a service provided to the participant who is coping with altered abilities and skills, a revision of long term expectations or changes in roles in relation to significant others.

Social Day Program “Without Walls”


The Self-Direct, Inc. Social Day Program “Without Walls” provides individually designed services, that take place in the community to improve and maintain the skills and abilities of participants to live as independently as possible in the community. The program will provide assistance, training, and supervision to individuals with varying abilities related to:

Personal Care Aide Certification Training Classes

In order to provide our consumers with the quality of care they deserve we need to hire and retain quality individuals. The employees who provide this service through Self-Direct Inc. must have completed a 40 hour classroom/clinical training in Personal Care. We at Self-Direct Inc. hire currently certified Personal Care Aides. This means they have a current NYSDOH certificate as a Personal Care Aide.
If a person has a HHA/PCA or CNA certificate and has not worked in the last year they can apply for our ACD (Alternative Competency Demonstration) training. This is a NYSDOH authorized program where a person who has been a Certified Nursing Assistant (CNA) or has worked as a home health aide (but not within the last two years), can take a test and demonstrate their skills in a classroom setting to a RN and become certified. This is an eight (8) hours class. They receive a certificate as a Personal Care Aide (PCA) upon completion of the class. Once the person has this certificate they can apply for a position at Self-Direct Inc.
Self-Direct Inc. also offers Personal Care Aide training. This is a 40 hour NYS DOH curriculum class that includes classroom training, demonstration, hands on and written exam. Upon successful completion of this course the person receives a NYSDOH certificate as a Personal Care Aide (PCA) upon completion of the class. Once the person has this certificate they can apply for a position at Self-Direct Inc.

Community Transitional Services (CTS)

Community Transitional Services (CTS) are defined as individually designed services intended to assist a participant to transition from a nursing home to living in the community.
This service is only provided when transitioning from a nursing home. These funds are not available to move from the participant’s home in the community to another location in the community.

Moving Assistance

Moving Assistance Services are individually designed services intended to transport a participant’s possessions and furnishings. This service can be used when the participant must be moved from an inadequate or unsafe housing situation to a viable environment which more adequately meets the participant’s health and welfare needs and alleviates the risk of unwanted nursing home placement. Moving Assistance may also be utilized when the participant is moving to a location where more natural supports will be available, and thus allows the participant to remain in the community in a supportive environment.

Respite Services

Respite Services is an individually designed service intended to provide relief to natural (informal), non-paid supports who provide primary care and support to a waiver participant. This is usually provided for participants who are in need of oversight and supervision as a discrete task. The primary location for the provision of this service is in the waiver participant’s home. Respite Services are provided in a 24-hour block of time.

Personal Emergency Response Systems (PERS)

A Personal Emergency Response System (PERS) is an electronic device designed to let you call for help in an emergency.

How a PERS Works

A PERS has three components: a small radio transmitter (a help button carried or worn by the user); a console connected to the user's telephone; and an emergency response center that monitors calls.

When emergency help (medical, fire, or police) is needed, the PERS user presses the transmitter's help button. It sends a radio signal to the console. The console automatically dials one or more pre-selected emergency telephone numbers. Most systems can dial out even if the phone is in use or off the hook. (This is called "seizing the line.") Most PERS are programmed to telephone an emergency response center where the caller is identified. The center will try to determine the nature of the emergency. Center staff also may review your medical history and check to see who should be notified. If the center cannot contact you or determine whether an emergency exists, it will alert emergency service providers to go to your home. With most systems, the center will monitor the situation until the crisis is resolved.

Environmental Modifications

Environmental modifications are internal and external physical adaptations to the participant’s home, that are necessary to ensure the health, welfare and safety of the individual, or which enable the individual to function with greater independence in and around the home and without which the participant would require institutionalization. All services shall be provided in accordance with NYS Uniform Fire Prevention and Building Code Sub-chapter E, and local building codes.

Modifications may also be made to a vehicle which is the waiver participant’s primary means of transportation when the vehicle is used to improve the waiver participant’s independence and inclusion in the community.
