Self-Direct Inc. of Baldwinsville was established in 2003 to provide services to individuals with Traumatic Brain Injury in the Central New York area. We are a N.Y.S. Licensed Home Care Agency committed to assisting individuals to remain independent in their own home and community.
Services are primarily through sources such as Home and Community Based Services, Medicaid Waiver for individuals with Traumatic Brain Injury (HCBS/ TBI), the New York State Department of Health (DOH), TBI Medicaid Waiver program, Onondaga County, Oswego County, as well as private insurance. Providers meet with the individual and their family to ensure that an individual’s plans are person-centered, incorporating the person’s preferences accordingly.
