individuals with different abilities. Service providers meet with the participant and their family to ensure that an
individual’s plans are person-centered, incorporating the person’s preferences accordingly. We assist and
support individuals and their families with care at home and full inclusion into the community. We provide
individuals with the opportunity to live independently with self-determination and respect. Our professional staff
assists our clients to obtain a sense of well-being and independence while providing safety and supervision
