Self-Direct Inc is a NYS Department of Health (DOH) Licensed Home Care Agency, established in 2003 with offices located in Baldwinsville and Utica to provide professional home care services and home medical equipment and supplies. We provide services under the NYS DOH Traumatic Brain Injury and Nursing Home Transition and Diversion waiver programs as well as working closely with insurance companies and private pay clients.
Our services revolve around person-centered practices putting our clients' needs first. We assist and support individuals and their families with care at home and full inclusion into the community. We provide individuals with the opportunity to live independently with self-determination and respect. Our professional staff assists our clients to obtain a sense of well-being and independence while providing health and safety oversight. Our services have grown to a full-service provider of home medical equipment and supplies for both the individuals we serve and the general public.

The Self-Direct, Inc. Structured Day Program Without Walls provides individually designed services, that take place in the community to improve and maintain the skills and abilities of participants to live as independently as possible in the community. Our professional staff assists our clients to obtain a sense of well-being and independence.
Our employees are N.Y.S. DOH screened with extensive background checks and references. All are trained, RN supervised and evaluated regularly in order to maintain our high standards of service and respect for our client. In order to provide our clients with the quality of care they deserve we need to hire and retain quality individuals.
Our experienced, professional staff provides the highest quality of care/service and are available for scheduled assistance 24 hours a day, 7 days a week. We recognize that our agency’s success is dependent upon our ability to provide a safe, reliable and respectful service.
