Since 2003, Self-Direct has been providing quality in-home supportive services to elderly, dementia, Alzheimer’s, and persons surviving a Traumatic Brain Injury (TBI).

We assist and support individuals and their families seeking care at home and/or in the community. We will maintain a plan of care that centers on the rights of the person we are serving to live as independently as possible, and continue to live their life where they wish.

We emphasize our coordination and communication with other healthcare providers to ensure early intervention steps are taken if necessary which in turn increases the quality of life for our clients and prevent possible hospital admissions.
